     My Choices
     An Advance Directive for Health Care Choices
 

LEI's Advance Care Planning Task Force created the My Choices advance directive booklet to provide Montana residents with information about one of life's most important activities: talking about and expressing one's end-of-life health care preferences. The booklet includes questions that can help people discuss their life values and goals with loved ones and others; questions to help people pick someone to represent them and make decisions if they are ever unable to do so; and resources to give them additional help.

The task force created, through a community consensus process, the accompanying advance directive form, to be used throughout Missoula County by adults of all ages.  This four-page form is written in plain English and conforms to Montana law.  It contains both the power of attorney for health care and living will in a single document.  My Choices has been adopted by health care facilities, attorneys, and faith communities throughout Missoula County.
